First Time Appointments
First Medical Massage Appointment: If your appointment is for Medical Massage,
you'll need to read, complete and sign the
Medical Intake, Policies & Rates forms to make sure you understand our policies and rates, and so we can
bill your insurance company correctly. Remember, you need a doctor's prescription with diagnosis code(s)
in order for us to provide you Medical (Insurance) Massage services and to bill your insurance correctly.
Please arrive 15 minutes early to allow enough time to complete the intake and insurance forms before your
session is scheduled to begin. Alternatively you may download these forms, print and fill them out prior
to the session, then bring them with you.
First Wellness Massage Appointment: Upon arrival to your first massage, we ask that you complete
a short
Client Intake form. The intake form allows us to get to know you, understand your unique health situation and
provide a safe and effective massage session. Please arrive 15 minutes early to allow enough time to complete
the intake before your session is scheduled to begin. Alternatively you may download this form, print and fill
it out prior to the session, then bring it with you.

if you need to cancel or reschedule your appointment.

Cancellation Policy
We understand that life can be full of surprises, which is why Living Lotus Massage
takes the time to provide an appointment reminder 24 hours before your appointment
(either by phone or soon by email as well). We ask the same from you in return.
Note: If you need to cancel an appointment, please please call or reschedule via our online appointment
system at least 24 hours in advance of your scheduled appointment time. Sudden absences without a cancellation
notice hurts our service quality and operation. Appointments cancelled less than 24 hours prior to the scheduled
appointment time will be subject to a charge equal to 50% of the massage service booked. Appointments not kept
will be charged 100% of the massage service booked. Even if the appointment was a Medical Massage appointment,
you will be responsible for payment out-of-pocket (your insurance company will not pay for your missed appointment).

Living Lotus Massage accepts a limited number of
3rd Party Coverage patients at any one time. If you are in a 3rd Party Coverage situation,
give us a call to see if we're able accomodate your situation.

3rd Party Coverage
3rd Party Coverage refers to the liability insurance coverage of the at-fault party.
If you don’t have PIP (Personal Injury Protection) coverage, or you’ve exhausted your
PIP benefits already (and you still need medical treatment) you are in a 3rd Party
Coverage situation.
Patients may then pursue the at-fault party’s insurance company to cover their ongoing
treatment. In many 3rd Party Coverage situations, healthcare professionals (massage
practitioners, chiropractors, physical therapists, etc.) who provide patients treatment
services may not be paid until that patient’s claim is completely settled. This can take
anywhere from six months to three years or longer (in WA state, you have a right to keep
your claim open up to three years after the date of the accident). In some cases health
care practitioners also run the added risk of not even receiving full payment for those
services, even after waiting up to three years.
Many massage clinics will not accept 3rd Party Converage patients because of the inherent
risks. We think it's important for people who find themselves in this position to have options
for receiving care, so contact us if this describes your situation. Living Lotus
Massage accepts a limited number of 3rd Party Coverage patients. To ensure we're fairly
compensated for delivering care under these uncertain payment conditions, we do require
that you read, understand and sign our 3rd Party Coverage Agreement prior to your first
appointment with us.

Medical (Insurance) Massage
Living Lotus Massage bases it's Medical Massage rates on the Usual, Customary and Reasonable (UCR)
fee schedule for the Seattle area and uses standard HCFA forms using Relative Value Unit (RVU)
pricing. RVU rates are:
- $30 per 15-minute unit for CPT code 97124 (massage)
- $30 per 15-minute unit for CPT code 97140 (manual therapy techniques)

Medical Massage Policies & Rates form above and bring it with you for your first appointment.
Wellness Massage
Single session rates:
- $45 - half hour session,
- $75 - one hour session,
- $110 - hour and a half session.
If you are receiving Wellness Massage (self-paid, not reimbursed by insurance) download and complete this
Wellness Massage Client Intake form above and bring it with you for your first appointment.
Beyond our single session rates, we also offer 5% or 10% discounted pre-paid
Wellness Packages.
Miscellanous Forms
If you are receiving an Energy System Consultation (self-paid, not reimbursed by insurance) download and complete this
Energy System Consultation Intake form above and bring it with you for your session.
Use this 3rd Party Coverage Agreement form only after talking with one of our practitioners about your 3rd party auto accident case.
Medical Records Rates
Copies of medical records are available (with signed consent of the patient) for 91 cents
per page for the first 30 pages, 69 cents per page for additional pages, and a $21 document
searching and handling fee. See the Washington Administrative Code
WAC 246-08-400 as well as the Revised Code of Washington
RCW 70.02.010 regarding these fees.
